Unfasten the tape or diaper cover, lift baby by his feet and use wipes to clean his bottom and then lower the baby back down onto the fresh diaper that's already in place. Apply ointment or powder if needed, and fasten the new diaper.
Baby boys will often urinate during a diaper change. Place a small cloth over the penis after unfastening the diaper to prevent a mess. Make sure to gently point the penis "south" when fastening the new diaper. This prevents urine from leaking out the top front of the diaper.
