1. Enlist the Help of Your Doctor
Your doctor can tell whether your chronic hepatitis C is undermining your quality of life. If this is the case, and you can't work, he may be able to help you get Social Security disability payments. To be eligible, your condition must make it impossible for you to work. You must show evidence that you have suffered from hepatitis C for one year, or the doctor must confirm that you will probably die from the condition.
2. Obtain Your Medical and Job History
To complete your application for disability, you will have to document when, where and by whom your chronic hepatitis C has been treated. List any medical tests you've undergone and medications you've taken. The government agency also will ask about your career history. Rather than filling these forms out from memory, ask your doctor for a copy of your file and use your updated resume to fill in the job information. Also, gather the following: a copy of your birth certificate, a copy of a W-2 form, Social Security numbers for everyone in your family and bank account numbers. You also may need to supply copies of your military discharge papers or worker's compensation documents, if applicable.
3. Schedule an Appointment
Once you've filled out your paperwork and gathered your forms, you must meet with someone from Social Security. That person will ask you more detailed questions about your medical and work history and answer any questions you might have about the process. Answer each question honestly; a false statement could jeopardize your claim. It typically takes 3 to 5 months for Social Security to make a decision.
