The DuoNeb nebulizer treatment is a combination of two drugs consisting of ipratropium bromide and albuterol sulfate in a single inhalation solution. DuoNeb is a bronchodilator used to treat obstructive airway diseases for patients who need more than one bronchodilator to manage their symptoms. Patients who are sensitive to albuterol, ipratropium bromide or atropine, should not use DuoNeb.
Nebulizer
A small volume nebulizer is a medical device that uses either compressed air, oxygen or an air compressor to convert the DuoNeb inhalation solution into aerosolized particles that can be inhaled into the lungs by using a mouthpiece or a mask attached to a nebulizer reservoir. A single dose of DuoNeb contains 3 mL of a combined solution of ipratropium bromide and albuterol sulfate.
Ipratropium Bromide
Ipratropium bromide is a long-acting bronchodilator used as a maintenance medication. According to the National Library of Medicine, DuoNeb has a 15-minute onset, with peak effectiveness in two hours, and lasts for up to five hours. Ipratropium is not a rescue medication and should not be used as one, particularly for acute asthma symptoms. Ipratropium bromide prevents wheezing, breathlessness, chest tightness, and coughing.
Albuterol Sulfate
Albuterol is a short-acting medication used to relieve reversible airflow obstruction brought on by acute symptoms of obstructive airway disease. Albuterol is considered a rescue medication for individuals experiencing bronchospasms or narrowing of the bronchial airways caused by inflammation associated with an allergic reaction to allergens or triggers such as pet dander, dust mites and smoking, to name a few.
In Combination
When combined, ipratropium bromide and albuterol sulfate can prevent wheezing, breathlessness, chest tightness and coughing. Individuals whose symptoms have not been successfully treated by ipratropium or albuterol alone, can use DuoNeb to successfully manage these symptoms. However, DuoNeb can cause significant cardiovascular side effects and should be used with caution. Recommended dosage is a single DuoNeb vial taken four times per day using a small volume nebulizer, with an additional two doses allowed daily if needed. DuoNeb is not recommended for children under 18 years of age.
