Pain in the upper right portion of the abdomen may have many causes. It may originate in the stomach, liver, gallbladder or pancreas. A problem with the heart or lung may be felt as abdominal pain. A musculoskeletal injury or condition could lead to pain in the area. Kidney stones may also lead to discomfort in this location.

Ulcers/GERD

An ulcer is an injury to the lining of the stomach. It may be a shallow divot or a pit that goes entirely through the stomach wall. Regurgitation of the stomach contents into the esophagus or food tube, which occurs in gastroesophageal reflux disease, may also cause pain in the right upper abdominal area.

Hepatitis

Inflammation of the liver from various causes is termed hepatitis. This may be caused by a virus, a drug or a toxin, such as alcohol. No matter the cause, hepatitis may lead to pain in the liver, which sits just under the ribs on the right side.

Gallbladder Disease

The gallbladder, which nestles into the liver, is a frequent source of right-sided abdominal pain. Cholecystitis, defined as inflammation of the gallbladder, may result from stone formation or an infection. An obstruction in the area from a tumor or growth may have the same result. At times, the gallbladder may not function properly and cause pain.

Pancreatitis

The pancreas is a gland that manufactures and releases insulin, among other hormones. Due to its anatomical location in the mid upper abdomen, the pancreas may lead to pain at the top, right portion of the belly. Gallstones may also obstruct the pancreas, causing pain and discomfort. A pancreatic mass or injury to the pancreas from alcohol or drugs could lead to pain in the area.

Referred Pain

Pain in the right lung, such as from pneumonia, may be felt in the upper abdomen due to the network of nerves in the area. Similarly, angina or a heart attack may lead to pain in the lower rib/upper abdomen area, though this is unusual.

Musculoskeletal Pain

A strained abdominal muscle, a fracture or injury to the pelvis, or a rib injury could cause pain at the top of the abdomen on the right side.

Kidney Stones

The kidneys rest deep in the abdomen, one on each side. A stone in the right kidney could send pain signals to the upper abdomen. Sometimes, stones migrate from the kidney into the ureter on their way to the bladder. This stretching and irritation of the ureter can be very painful.

Shingles

Herpes zoster, also known as shingles, can cause an uncomfortable rash in any area of the body. It's localized to one side and is usually preceded by a burning discomfort. Often, the cause of this mysterious pain is not seen until the rash forms after several days.
