How to Change Your Last Name to Your Husband's Last Name

After marriage, each individual has the option to take his or her spouse's last name. Traditionally, however, women take the last names of their husbands. If you desire to change your last name, you can officially change it with the Social Security Administration. They will issue you a new social security card, which you can use to change your last name on other documents and forms of identification.

Step 1

Obtain and complete an SS-5 application. You can obtain this application from the nearest Social Security Administration office. You can also visit the Social Security Administration website. If you print and complete the application from the website, you will need to send the application in the mail to any Social Security Administration office. Indicate your new and old last names on the application.

Step 2

Supply the Social Security Administration with evidence of your marriage and your identity. You should supply them with your marriage license and your driver's license, identification card or passport. Include original documents with the application if you are sending it in the mail. They return the documents to you once the office completes your request.

Step 3

Visit your local Department of Motor Vehicles (DMV) to change your last name on your driver's license and other motor-related documentation. Depending on your state, the application you will need to complete varies. You will need to supply your social security number and marriage license as proof of your legal name change. Make sure your name change is official with the social security Administration before visiting the DMV.

Step 4

Update your last name with your financial institutions. Visit the institutions and alert them of your name change. You will need to supply them with a copy of your new social security card or some other form of identification. You will need to order new credit or debit cards to reflect your new last name.

Step 5

Let people know about your new last name. You will need to alert your job to change your name for both financial and social purposes. Let your friends and acquaintances know what last name they should refer you to.
