Accredited by the Joint Commission and Medicare and Medicaid certified, Sunrise Home Health Care Inc. is a Miami, Florida-based agency that provides at-home medical service as an alternative to inpatient care at a hospital, nursing home or center of rehabilitation. Home health care providers work in tandem with physicians in treating patients to ensure continuity of care between medical appointments.

History

Established in 1992 in Florida's Miami-Dade County, Sunrise Home Health Care Inc.'s mission statement expresses a commitment to provide both physical care and emotional support to home-bound individuals. "Elite" magazine recognized the company's goal in 2008 when it ranked Sunrise Home Health Care as one of the top 500 home health care agencies in the nation.

Staff

Sunrise Home Health Care Inc.'s staff includes occupational and speech therapists, registered nurses, licensed practical nurses, physical therapists, home sitters, nutritionists, social workers and home health aides. The company appoints staff based on patient need and, as their roles suggest, serve different purposes. All staff are subject to a background check and verification of state licensing.

Services

Sunrise Home Health Care Inc. provides patients with PEG tube feeding, speech therapy, diabetes management, patient education, phlebotomy, wound care, physical therapy, foley catheter maintenance, nutrition service and diet management, ostomy care, IV therapy, occupational therapy and medication management. Nonclinical services offered are the ordering of laboratory studies and medical supplies and arranging the delivery of medical durable equipment to patients' homes. Medicare and Medicaid may authorize payment for any of these.

Medicare and Medicaid Stipulations

Patients must meet specific criteria for Medicare or Medicaid to cover payment for home health care. A physician must certify home confinement. Confinement may be necessary because of an illness, injury or recovery from surgery. Allowable home absences are those for additional medical treatment. Further, Medicare and Medicaid will only pay for physician-ordered treatments and prescriptions. Patients bound to assisted living facilities as opposed to private homes are also covered for service if the facility does not provide a needed service.

Nursing Home Diversion Program

Sponsored by Florida's Department of Elder Affairs, Sunrise Home Health Care operates a program designed to prolong the quality of life for elderly patients to avoid transition to a nursing home. Nursing home diversion program participants are Miami-Dade residents who are at least 65 years old and enrolled in Medicare parts A and B. Participants must also be Medicaid eligible. The Department of Elder Affairs' CARES unit determines ultimate eligibility. In addition to physician-ordered services, participants enjoy home delivered meals, chore services, dental, hearing and vision care, and transportation arrangement all at no additional cost to them.
