Underground pimples get their name because they're located deeply under the skin. Unlike whiteheads, blackheads or pustules, underground pimples may take weeks to heal and are often extremely painful. Acne.org characterizes underground pimples as large, hard bumps that shouldn't be squeezed or picked at. Doing so can push the infection deeper under the skin, resulting in more pain and potential permanent scarring.
Step 1
Cleanse your face with tepid water and a mild acne cleanser and then gently pat dry. Avoid scrubbing or using a rough exfoliant on painful acne.
Step 2
Soak a clean washcloth in hot water and place it directly over the underground pimple.
Step 3
Hold the hot towel over the pimple for 30 or more minutes and re-soak the towel often to keep it hot.
Step 4
Apply a small amount of topical acne treatment to the tip of a cotton swab. Dab the ointment directly to the underground pimple until it's covered completely.
Step 5
Repeat no more than twice daily until the underground pimple goes away.

- Consult a doctor or dermatologist about prescribed acne medication for persistent or painful underground pimples.
- Do not apply heavy cosmetics, creams or lotions to the underground pimple.
