DR. CAMERON CRAVEN: Hi. I'm Dr. Cameron Craven from Westlake Dermatology and Cosmetic Surgery, and I'm going to give you a general overview of blepharoplasty or eyelid surgery, which is designed to rejuvenate and restore the appearance of the eyelids. Starting with the upper eyelids, as the upper eyelids age, they tend to accumulate excess skin with or without bulging fat that can tend to hangover the eyelashes and occasionally obstruct vision. In general, to correct this condition, it is helpful to remove the excess skin and fat occasionally with a small amount of muscle from the upper eyelid. The amount of fat resected will depend on your clinical indications and oftentimes, it is preferable not to remove any fat from the upper eyelids. With regard to the lower eyelids, the incisions can be placed either through the inside of the eyelid or on the outside of the eyelid. Incisions through the inside of the eyelid may result in no perceptible external scars; however, they do not allow for any resection or removal of excess skin. Incisions on the outside of the eyelids may result in perceptible scars for a period of time until they fade, but do allow for removal of skin directly and may be more appropriate for patients with more significant excess skin. Additionally, fat may be removed or preferably repositioned in lower eyelid surgery. Oftentimes, upper and/or lower eyelid surgery are combined with other adjunctive procedures to help improve or enhance cosmetic outcomes. This will include chemical peels or laser resurfacing.
